ABSTRACT

Abstract This study explores the antioxidant activity, phytochemical screening, total phenolic and flavonoids contents in the extracts of four locally available weeds plants namely Convolvulus arvensis, Chenopodium murale, Avena fatua and Phalaris minor with different solvents. The antioxidant activities of these extracts were determined via various in-vitro methods such as total antioxidant activity (TAA), reducing power (RP), DPPH (2,2-Diphenyl-1-Picrylhydrazyl) free radical scavenging and hydrogen peroxide scavenging assays. Phytochemical screening was performed both qualitatively as well as quantitatively. Total phenolic content (TPC) and total flavonoid content (TFC) were determined through Folin- Ciocalteu reagent and aluminium chloride methods respectively. Methanol-chloroform solvent showed the presence of a high amount of TPC in milligram of gallic acid equivalent per gram of dry weight (mg of GAE/g of DW) in the extracts of all weeds. Their descending sequence was Avena fatua (74.09) Phalaris minor (65.66) Chenopodium murale (64.04) Convolvulus arvensis (61.905), while, chloroform solvent found to be best solvent for the extraction of TFC. Methanol-chloroform solvent was also found to be best solvent for TAA (Total antioxidant activity assay) which showed values in milligram of ascorbic acid equivalent per gram of dry weight (mg of AAE /g of DW), for DPPH scavenging activity, reducing power (antioxidant activity) and hydrogen peroxide scavenging activity. Phytochemical screening indicated the presence of polyphenols, flavonoids, tannins, saponins, alkaloids and glycosides in these weeds.

ABSTRACT

Distribution pattern and relative abundance of the agricultural rodent pests of rain-fed and irrigated areas of district Swat Khyber Pakhtunkhwa, Pakistan were recorded from April 2011 to November 2013 using wire mesh live traps. A total of 350 rodents (269 Rattus rattus and 81 Mus musculus) were captured under 2268 trap nights (trap success: 0.15). Regression of daily captures on cumulative captures revealed an estimate of 350 rodents from all the sampled structures with an average of 21.8 rodents per crop field. House rats (R. rattus; 76.8% of captures) were numerically dominant in almost all types of agricultural fields, and were significantly different from the mice (Mus musculus; 23.1% of captures). Both species were found together in some fields. The sex ratio revealed that males outnumbered the females in both of the reported species.

ABSTRACT

Malakand region is an endemic area for cutaneous leishmaniasis (CL). However, there are limited number of studies of this disease in Pakistan. Therefore, a study was conducted to understand the level of awareness attitude and practice among the residents of Makaland towards CL and the disease vectors. This study adopted a cross-sectional approach with a total of 400 respondents (n=93 rural and n= 307 urban). Overall, the population in Malakand region (61.2%) were well-informed in the role of sand fly in transmitting diseases, but most lack knowledge on the vector's behavior and almost a quarter (24.5%) were unable to provide knowledge on proper control measures. Alarmingly, the practice and attitudes of the general population was not satisfactory as close to half (49.8%) of the population did adopt any control method. This study calls for increase in awareness through health education campaign to reduce the risk of cutaneous leishmaniasis outbreaks in the future.

ABSTRACT

Abstract Plant growth regulators and improved planting density are the innovative techniques in the establishment of more productive cotton crop. A field study was planned to assess the role of growth regulators in the resource utilization efficiency of cotton cultivars under different row spacing at Agronomic Research Area, Bahauddin Zakariya University Multan and Usmania Agricultural Farm, Shujaabad during Kharif 2012. The study was comprised of cotton cultivars viz. CIM-573 and CIM-598, cultivated under conventional (75 cm), medium (50 cm) and improved ultra-narrow row spacing (25 cm) and foliar spray of growth regulators viz. moringa leaf extract (MLE) and mepiquat chloride (MC), either alone or in combination, distilled water as a control. The application of MLE alone and in combination (MLE + MC) showed the promoting effect on crop growth rate, net assimilation rate, leaf area index, leaf area duration, sympodial branches and number of bolls leading to higher seed cotton yield of both cotton cultivars grown under conventional row spacing. While application of MC averts the plant growth without considerably improving the productivity. MLE being rich source of growth promoting hormone and nutrients showed its potential to a far greater extent under conventional row spacing in efficient utilization of available resources compared to MC and distilled water.

Routine ilioinguinal nerve excision has been proposed to avoid chronic post-hernioplasty neuralgia. The study aimed to evaluate the effect of routine ilioinguinal nerve excision compared to nerve preservation on chronic groin pain and other sensory symptoms in lichtenstein inguinal hernia repair. Methods: The study was conducted in the department of general surgery, at S.P. Medical College and PBM Hospital, Bikaner for duration of 15 months from august 2018 to November 2019. A total of 60 patients of uncomplicated inguinal hernia, who met the inclusion criteria, planned for lichtenstein hernioplasty were randomly divided into 2 groups with 30 cases with ilioinguinal nerve preservation (group A) and 30 cases with elective division of the nerve (group B). Follow up was done upto1 year and patients were inquired regarding pain, hypoesthesia and numbness and data was compared and analysed. Results: The incidence of post-operative neuralgia was 23.33% vs 6.67% (p=0.05) at 1 year in group A and group B respectively. The difference in pain was significant in both groups. The incidence of post-operative hypoesthesia was 10% vs 16.67% (p>0.05) at 1 year follow up in group A and group B respectively. The incidence of numbness was 11.11% vs 16.67% (p>0.05) at 1 year in group A and group B respectively. No statistically significant difference was found in both groups. Conclusions: The prophylactic exicision of ilioinguinal nerve during lichtenstein mesh repair decreases the incidence of chronic groin pain after surgery.

ABSTRACT

Introduction: Pseudocyst of the pinna is a rare condition that occurs when fluid accumulates in the intercartilagenous space of the auricle. The main goal when treating this condition is to keep acceptable cosmetic results with no recurrence rate. Objective: To demonstrate the superior impact of incision and drainage of auricular pseudocysts with the insertion of a catheter drain and daily irrigation as the treatment of this condition. Methods: A total of 42 patients with auricular pseudocysts were enrolled in the present study in the period between May 2011 and May 2017. All of the patients were treated with incision and drainage with compression. The average follow-up time reached ~ 6 months in all of the cases. Results: All of the patients had satisfactory cosmetic results with no recurrence and no complications. Conclusion: Among the different methods of surgical treatment of pseudocyst of the pinna, incision and drainage with daily irrigation is a significantly efficientmethod both for the eradication of auricular pseudocysts and for good cosmetic results (AU)

ABSTRACT

Inhaled foreign body (FB) is one of the most problematic conditions, especially in childhood. Rapid and safe bronchoscopy removal of inhaled foreign bodies is the treatment of choice in these cases. Spontaneous expulsion of inhaled FB bronchus is rarely mentioned in the literature. We present three cases of expelled foreign bodies of which two were coughed out, and the third one went into the gastrointestinal tract. Spontaneous expulsion of inhaled FB though is rare, can save the patient and relieves the condition, but it carries a high risk of possible FB entrapment in the larynx that may lead to patient’s death.

ABSTRACT

Objectives: the operating theatre [OT] is a vital facility that utilises a considerable portion of the hospital's budget; thus proper OT utilisation is essential. Surgical cancellation is a leading cause of OT underutilisation. This study aimed to report the rate and reasons for surgical cancellations and no-shows in a large regional hospital in Oman

Methods: this study took place as part of a retrospective quality management project at the Ibri Regional Hospital, Ibri, Oman. All elective surgical procedures scheduled between January and December 2014 were included. Cancelled procedures were reviewed to determine the reasons for cancellation

Results: a total of 4,814 elective procedures were scheduled during the study period; of these, 1,235 [26%] were cancelled. Patient no-shows were the most prevalent reason for surgical cancellation [63%], followed by surgical reasons [17%]; in contrast, OT-associated reasons were responsible for only 2% of cancellations. According to speciality, general surgery had the highest percentage of total cancellations [65%], while ear, nose and throat had the highest rate of surgical cancellations among their scheduled cases [42%]

Conclusion: Ibri Regional Hospital had a higher surgical cancellation rate due to no-shows than those reported in the literature. Regular audits, quality management projects and the appointment of a dedicated procedure booking coordinator may enhance proper utilisation of the OT, potentially saving funds, conserving resources and alleviating the burden of cancellations

ABSTRACT

Infestation with leeches can happen due to drinking from spring water or other unhealthy water from non-hygienic sources or swimming in infested water like Wells and swamps, and can lead to several complications in the aerodigestive tract. Epistaxis, melena, and foreign body sensation had been reported in the literature as manifestations of this condition. We described a 12-year-old child presented to our department with dry cough and spitting of blood after swimming in the water pond of a Well that led to leech entry into the nasopharynx. Removal was done, and the patient became symptom free. Infestation with a leech can lead to variable symptoms in the aerodigestive tract like spitting of blood and irritation at the site of its attachment. Hence, leech should be put in mind in dealing with patients visiting physicians with the above symptoms, especially, if they come from rural areas that drink spring or Well water.

ABSTRACT

To assess the priority of various aspects of the patient-primary care physician relationship in the decision to visit again that same physician. A total of 400 community residents in Ras Beirut, Lebanon. Study design: A cross-sectional community based study sampled by a nonrandom sex-education quota-based procedure. Participants were asked to fill a survey where they indicated the ranking of nine items by importance in their decision to revisit the same physician. The nine items were chosen from three categories of factors: professional expertise of the physician ; characteristics of the patient-physician relationship, office organization. Having a physician that gives the patient adequate time for discussion prevailed as rank 1 and luxurious clinic ranked as 9th. Affordability was one of the main concerns among men, those with poor health and those of lower socioeconomic status. Accessibility of the physician's phone was considered highly important among women and those of lesser education status. This study emphasizes the importance of adequate time with the patient, accessibility and affordability of the physician in maintaining continuity of care and patient satisfaction, beyond mere medical expertise

ABSTRACT

Interrupted Aortic Arch (lAA) is the absence or discontinuation of a portion of the aortic arch. There are three types of Interrupted Aortic Arch, and they are classified according to the site of the interruption. lsolated IAA appears commonly in type B lAA,where the interruption occurs between the left carotid artery and the left subclavian artery. In such case there is no associated cardiac anomaly and the patient may not exhibit lethal form of presentation unless there is duct dependent circulation or ventricular septal defect. lsolated interrupted arch is relatively rare congenital anomaly and very few cases were reported in literature so far.

ABSTRACT

Diabetes mellitus is a world-wide disease associated with many complications among which is cardiovascular autonomic nervous system dysfunction and divers group of other systemic involvements mainly retinopathy, nephropathy, dermopathy by micro- and macrovascular unique angiopathies and there is no organ in the body that is immune against this metabolic and vascular disorder. The aim of this study is to assess the incidence and pattern of cardiovascular autonomic dysfunction in type 2 diabetic patients and its possible association to patient's age, sex, duration of disease and its 5 relation to diabetic nephropathy and retinopathy. This is a cross sectional study of 50 type 2 diabetic patients and 50 healthy controls between age of j 40 and 55 years. After full history, complete clinical examination, laboratory analysis did which included complete blood count, random blood sugar, general urine examination, and 24 hour urine collection for protein to assess nephropathy. Both direct and indirect fundoscopy conducted by ophthalmologist after papillary dilatation to check for retinopathy. Cardiac autonomic dysfunctions were checked and categorized as normal, parasympathetic, sympathetic, and mixed pattern using 5 standard technique described by Ewing et al. Thirty one [62%] diabetic patients have cardiac autonomic dysfunction compared with none [0%] of controls [p=value was 0.000000001] was highly significant and the most common autonomic dysfunction pattern was the mixed pattern 32 [64%] patients. The patients showed a significantly higher resting rate [89.7 +/- 7.7 in female and 88 +/- 7.4 in male] compared to the controls [75.2 +/- 7.2 in female and 74.1 +/- 5.4 in male]. Diabetic nephropathy 23 patients [46%] or retinopathy 21 patients [42%] had a significantly higher prevalence of diabetic autonomic neuropathy than diabetic patients without these complications. The patients with sympathetic and parasymathatic autonomic neuropathy has significantly longer duration of diabetes mellitus [9.8 +/- 1.8, and 8.42 +/- 2.19 years respectively] compared to patient with no autonomic dysfunction [4.9 +/- 1.3 and 3.9 +/- 0.6 years respectively]. The incidence of autonomic dysfunction in diabetic patients using non-invasive tests is high [62%] and the most common pattern was the mixed pattern [32%]. Also there was significant association 1 between autonomic dysftmction, duration of the disease and autonomic nervous system symptoms
